One time enrollment fee of $79.
Those who qualify receives one whitening starter kit (includes one set of take home trays with two bleaching gels) for FREE* once they had a completed new patient comprehensive exam, x-rays and cleaning. Then come in every 6 months for your regular completed check-up and cleaning and receive another FREE bleaching gel to keep your smile white and bright.
*To receive the free custom made take home trays with bleaching gels,
patient must be free of: active decay and non-restorable teeth; periodontal/gum
disease; oral cancer, etc.
† Patient of records who wish to join must have an update exam,
x-rays and cleaning if they have not been seen in the past 6 months and
be free of active decay, all non-restorable teeth treated, periodontal/gum
disease completed, etc.
